Contact E-mail: tmj92@txstate.edu; shahid.shah@netspective.com Provisional Patent Application Filed: 2026-004 Innovation Summary: Care coordination involves facilitating patient-centered care among various parties consisting of caregivers, primary care practitioners, and specialists. Services included in care coordination assist with transition of care from pediatric to adult care and facilitate inpatient (i.e., major surgeries) and outpatient procedures (i.e., minor surgeries). Additionally, care coordination involves sharing educational resources, communicating with various providers, developing tailored treatment plans, and monitoring and following up on patients’ health progress. Individuals who are at-risk or currently living with chronic health conditions such as Prediabetes, Type 1 Diabetes, Type 2 Diabetes, and Gestational Diabetes are at a higher risk of developing additional chronic health conditions and experiencing serious medical complications. Therefore, it is essential for this particular patient population to remain informed about their health condition(s), feel confident in preventing or managing their condition(s), and to receive optimal healthcare service delivery. However, patients, caregivers, and providers have expressed concerns with the care coordination process based on low health literacy, limited resources, and time constraints. Our digital educational platform will serve as a comprehensive innovation to support patients, caregivers, and healthcare professionals with care coordination resources that are interactive and tailored based on specific needs. Additionally, our innovation will provide educational modules containing care coordination content for medical students, medical interns, and medical residents since it is essential for these future healthcare providers to learn more about the processes of facilitating medical care efficiently before, during, and after the conclusion of their training. Consumers will have an interest in this innovation because this digital educational platform will serve as a comprehensive, long-term resource for patients, caregivers, and healthcare professionals by incorporating resources into one platform. Additionally, maintaining this innovation will be cost-effective and will facilitate a seamless, well-structured care coordination, which can lead to reducing healthcare expenditures and decreasing hospital readmission rates. Our overall goal is to increase health literacy, improve patient health outcomes, and create structured care coordination procedures.
